SO!, after Sherlock has “fallen” this is him being wheeled away by the hospital staff, but look at that face and distinct lack of cheekbones (shut up with your logic that the fall would have crushed them they are just to pronounced to quit).  So somehow the hospital staff is in on it?  This photo does not do justice to this claim, but see for yourself, the time stamp  is 1:23:07. funny enough, this person kind of looks like mycroft if you stare long enough - the nose.
